Line Throwing Apparatus observation - missed expiry date

What happened

During the third-party inspection, it was found that all four Line Throwing Apparatuses (LTA) were expired.

Root cause

An investigation unveiled that responsible officer (3rd Officer) wasn't aware that there are two expiry dates: one for rocket and one for the casing. He used to check only casing expiry date.

Corrective actions

1. Equipment manual should be studied prior maintenance and checking.
2. New and inexperienced personnel should be trained and guided if lack of knowledge observed.
