During rough sea condition the ME FO pressure was dropped by the CE order to clean the FO Duplex filter. While opening the cover of FO filter the fuel oil came out with high pressure and temperature. Crew hurried up to close the cover and check the position of change over valve and found out that the position of the change over lever was not correct.
Hurry to complete the job caused lack of attention during preparation activities.
In any circumstances prior commencing any job proper preparations, isolation and locking out activities to be done.
Always check presence of pressure inside the filter body prior to opening. Use air drain cock/plug.
