  • Unfamiliar with operating/calibrating procedures

Emergency Shutdowns

Observations:

  • Painted fusible elements

MARVS - Maximum Allowable Relief Valve Settings

Observations:

  • No tampered, missing, painted over, or worn seals

  • No setting certification record

Deck Water Spray

Observations:

  • Obstructed nozzles

  • Insufficient water pressure

Electrical equipment (On deck in the gas dangerous zone)

Observations:
